Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks the vasoconstrictor effects of angiotensin II
- Has more than a 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor
- Does not inhibit ACE (kininase II)
- Does not affect the response to bradykinin
Dosage & Administration
- Usual recommended starting dose is 20 mg once daily
- Dose may be increased to 40 mg for patients requiring further reduction in blood pressure
- Twice-daily dosing offers no advantage over once daily dosing
- No initial dosage adjustment recommended for elderly patients or patients with moderate to marked renal impairment
- May be administered with or without food
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by co-administration of antacids
- Not metabolized by the cytochrome P450 system
- Interaction with drugs that inhibit, induce, or are metabolized by those enzymes are not expected
- Possible deterioration of renal function with co-administration of NSAIDs including selective COX-2 inhibitors
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well-tolerated with an incidence of adverse reactions similar to placebo
- No dose-related frequency of adverse reactions
- Common adverse reactions include dizziness, back pain, bronchitis, diarrhea, headache, hematuria, hyperglycemia, hypertriglyceridemia, and others
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Dangers associated with use during the second and third trimesters
- Potential excretion in human milk
Precautions & Warnings
- Changes in renal function may be anticipated in susceptible individuals treated with Olmesartan
- Treatment may lead to oliguria and/or acute renal failure in certain patients
Overdose Effects
- No experience of overdose reported
- Likely effects include hypotension and tachycardia
