Therapeutic Class

  • Angiotensin-ll receptor blocker

Indications

  • Treatment of hypertension
  • May be used alone or in combination with other antihypertensive agents

Pharmacology

  • Blocks vasoconstrictor effects of angiotensin II
  • Inhibits negative feedback of angiotensin II on renin secretion

Dosage & Administration

  • Usual recommended starting dose: 20 mg once daily
  • Dose may be increased to 40 mg for further reduction in blood pressure
  • No initial dosage adjustment recommended for elderly patients
  • No initial dosage adjustment recommended for patients with moderate to marked renal impairment
  • May be administered with or without food

Interaction

  • No significant drug interactions reported with digoxin or warfarin
  • Bioavailability not significantly altered by antacids
  • Not metabolized by the cytochrome P450 system
  • Potential deterioration of renal function when co-administered with NSAIDs
  • Antihypertensive effect may be attenuated by NSAIDs

Contraindications

  • Hypersensitivity to any component of the product

Side Effects

  • Well tolerated with an incidence of adverse reactions similar to placebo
  • Incidence of adverse reactions not dose related
  • Most common adverse reaction: dizziness
  • Other adverse reactions at more than 1% incidence: back pain, bronchitis, increased creatine phosphokinase, diarrhea, headache, hematuria, hyperglycemia, hypertriglyceridemia, influenza-like symptoms, pharyngitis, rhinitis, and sinusitis
  • Incidence of cough similar to placebo

Pregnancy & Lactation

  • Discontinue when pregnancy is detected
  • Can cause injury or death to developing fetus when used during second and third trimesters
  • Potential excretion in human milk

Precautions & Warnings

  • May cause changes in renal function
  • Association with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals

Overdose Effects

  • No experience of overdose reported
  • Most likely effects: hypotension and tachycardia
  • Management may include gastric lavage, induction of emesis, and cardiovascular system support
