Therapeutic Class

Indications
- Treatment of hypertension
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding to AT 1 receptor
- No effect on ACE or bradykinin response
- Inhibition of negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Usual starting dose of 20 mg once daily
- May be increased to 40 mg for further blood pressure reduction
- No advantage with twice-daily dosing
- No initial dosage adjustment for elderly patients or those with moderate to marked renal/hepatic impairment
- Consideration for lower starting dose in patients with possible depletion of intravascular volume
- May be administered with or without food
Interactions
- No significant drug interactions reported
- Bioavailability not altered by antacids
- Not metabolized by cytochrome P450 system
- Potential renal function deterioration with NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well-tolerated with incidence of adverse reactions similar to placebo
- Frequency of adverse reactions not dose-related
- Withdrawal rate due to adverse reactions at 2.4%
- Adverse reactions include dizziness, back pain, diarrhea, headache, hyperglycemia, pharyngitis, rhinitis, sinusitis, cough
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Potential injury to developing fetus during second and third trimesters
- Potential adverse effects on nursing infant
Precautions & Warnings
- May cause changes in renal function
- Associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- Hypotension and tachycardia likely effects of overdose
- Bradycardia possible with parasympathetic stimulation
- Gastric lavage or induction of emesis may be considered for recent intake
- Clinically significant hypotension requires active cardiovascular system support
