Indications
- Treatment of hypertension
- Can be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding to AT 1 receptor in vascular smooth muscle
- Higher affinity for AT 1 receptor than for AT 2 receptor
- Does not inhibit ACE (kininase II)
- Does not affect the response to bradykinin
- Inhibits the negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Recommended starting dose: 20 mg once daily
- May increase to 40 mg for further reduction in blood pressure
- No additional benefit in twice-daily dosing
- No initial dosage adjustment recommended for elderly patients or those with moderate to marked renal/hepatic dysfunction
- Consider lower starting dose for patients with possible depletion of intravascular volume
- May be administered with or without food
- Safety and effectiveness in pediatric patients not established
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by cytochrome P450 system
- No effect on P450 enzymes
- Potential deterioration of renal function with NSAIDs or selective COX-2 inhibitors
- Attenuation of antihypertensive effect by NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well-tolerated with incidence of adverse reactions similar to placebo
- Overall frequency of adverse reactions not dose-related
- Withdrawal rate due to adverse reactions: 2.4%
- Dizziness as the most common adverse reaction
- Other adverse reactions include back pain, bronchitis, diarrhea, headache, hyperglycemia, rhinitis, etc.
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Can cause injury and death to developing fetus during second and third trimesters
- Potential secretion in low concentration in the milk of lactating rats
- Adverse effects on nursing infant possible
Precautions & Warnings
- Inhibiting the renin-angiotensin-aldosterone system may lead to changes in renal function
- Oliguria, progressive azotemia, acute renal failure, and death reported
- Similar results anticipated in patients treated with Olmevas
Overdose Effects
- No reported experience of overdose
- Likely effects: hypotension, tachycardia, bradycardia
- Management: active support of cardiovascular system, close monitoring, elevation of extremities, attention to circulating fluid volume and urine output
