Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding to the AT 1 receptor in vascular smooth muscle
- More than 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- No initial dosage adjustment recommended for elderly patients
- May be administered with or without food
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Does not inhibit cytochrome P450 system
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well-tolerated treatment with an incidence of adverse reactions similar to placebo
- Most common adverse reaction: dizziness
- Other adverse reactions occurred at an incidence of more than 1% and similar to or greater than placebo
Pregnancy & Lactation
- Discontinue if pregnancy is detected
- Drugs acting on renin-angiotensin system can cause injury to developing fetus, potential excretion in human milk
Precautions & Warnings
- Inhibiting the renin-angiotensin-aldosterone system may cause changes in renal function
- Treatment associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose reported
- Likely effects: hypotension, tachycardia
- Clinically significant hypotension requires active cardiovascular support
