Indications

  • Treatment of hypertension
  • May be used alone or in combination with other antihypertensive agents

Pharmacology

  • Blocks vasoconstrictor effects of angiotensin II
  • Selective blocking of the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
  • No inhibition of ACE (kininase II)
  • Does not affect the response to bradykinin
  • Inhibits negative regulatory feedback of angiotensin II on renin secretion

Dosage & Administration

  • Usual recommended starting dose is 20 mg once daily
  • Dosage must be individualized
  • Dose may be increased to 40 mg for further reduction in blood pressure
  • Twice-daily dosing offers no advantage over the same total dose given once daily
  • No initial dosage adjustment recommended for elderly patients
  • Recommended to administer with or without food

Interaction

  • No significant drug interactions reported with digoxin or warfarin
  • Bioavailability not significantly altered by antacids
  • Not metabolized by the cytochrome P450 system
  • No effects on P450 enzymes
  • Potential renal function deterioration with NSAIDs and COX-2 inhibitors

Contraindications

  • Contraindicated in patients hypersensitive to any component

Side Effects

  • Well tolerated with an incidence of adverse reactions similar to placebo
  • Incidence of adverse reactions not dose related
  • Common adverse reactions include dizziness, back pain, diarrhea, headache, hyperglycemia

Pregnancy & Lactation

  • Discontinue use when pregnancy is detected
  • Can cause injury and even death to the developing fetus during the second and third trimesters
  • Not known whether excreted in human milk

Precautions & Warnings

  • Changes in renal function may be anticipated in susceptible individuals
  • Associated with oliguria, progressive azotemia, acute renal failure, and death in some cases

Overdose Effects

  • No experience of overdose reported
  • Most likely effects are hypotension and tachycardia
  • Possible encounter with bradycardia
