Indications

  • Treatment of hypertension

Pharmacology

  • Olmesartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin-II
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage & Administration

  • Usual starting dose of 20/12.5 mg once daily
  • May be titrated at intervals of 2-4 weeks

Interaction

  • Olmesartan has no significant drug interactions reported
  • Hydrochlorothiazide may interact with alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, corticosteroids, ACTH, and lithium

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria
  • Hypersensitivity to sulfonamide-derived drugs

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Monitoring for electrolyte imbalance and hypokalemia
  • Caution in patients with impaired renal function

Use in Special Populations

  • Recommended regimens for renal impairment patients with creatinine clearance >30 ml/min
  • Preferential use of loop diuretics over thiazides in severe renal impairment
  • Not recommended for patients with severe renal impairment
  • No dosage adjustment necessary with hepatic impairment
  • Safety and effectiveness not established in pediatric patients
  • Caution in dose selection for elderly patients

Overdose Effects

  • Manifestations of olmesartan overdose: hypotension and tachycardia
  • Manifestations of hydrochlorothiazide overdose: electrolyte depletion, hypokalemia, hypochloremia, dehydration
