Indications

Olmesartan Medoxomil & Hydrochlorothiazide combination is indicated for the treatment of hypertension.

Pharmacology

  • Olmesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin-II by selectively blocking the binding of angiotensin-II to the AT 1 receptor
  • Hydrochlorothiazide is a thiazide diuretic that affects the renal tubular mechanisms of electrolyte reabsorption

Dosage & Administration

The usual starting dose is 20/12.5 mg one tablet once daily. Dosing should be individualized.

Interaction

  • Olmesartan: No significant drug interactions were reported
  • Hydrochlorothiazide:
    • Alcohol, barbiturates, or narcotics may potentiate orthostatic hypotension
    • Dosage adjustment of antidiabetic drugs may be required when administered with hydrochlorothiazide

Contraindications

The combination is contraindicated in patients hypersensitive to any component of the product

Safety and effectiveness in nursing mothers and pregnancy have not been established

Precautions & Warnings

  • Periodic determination of serum electrolytes should be performed
  • Hyperuricemia may occur in certain patients receiving thiazide therapy
  • Impaired renal function should be considered

Use in Special Populations

  • Renal Impairment Patients: The usual regimens of therapy may be followed provided the patient's creatinine clearance is >30 ml/min
  • Hepatic Impairment Patients: No dosage adjustment is necessary
  • Paediatric use: Safety and effectiveness in paediatric patients have not been established
  • Geriatric use: Dose selection for an elderly patient should be cautious

Overdose Effects

  • Olmesartan: Limited data are available in regard to overdosage
  • Hydrochlorothiazide: The most common signs and symptoms observed are those caused by electrolyte depletion
