Indications
- Treatment of hypertension alone or with other antihypertensive agents
- Lowering blood pressure
- Initial therapy in patients likely to need multiple antihypertensive agents
- Individualized blood pressure goals
Pharmacology
- Dihydropyridine calcium channel blocker
- Inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
- Peripheral arterial vasodilator
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
Dosage & Administration
- Initiate with 5/20 mg once daily for 1 to 2 weeks
- Titrate as needed up to a maximum of 10/40 mg once daily
- Recommended starting dose of Amlodipine is 2.5 mg in patients 75 years old
- Not recommended for patients >75 years old
Interaction
- Antihypertensive effect may be attenuated by NSAIDs including selective COX-2 inhibitors
- Close monitoring of blood pressure, renal function, and electrolytes
Contraindications
- Cannot be co-administered with Aliskiren in patients with diabetes

Pregnancy & Lactation
- Pregnancy Category D
- Not recommended in 2nd and 3rd trimester
- Potential adverse effects on nursing infant
Precautions & Warnings
- Caution in volume- or salt-depleted patients
- Risk of hypotension
- Risk of vasodilation in patients with severe aortic stenosis
- Risk of increased frequency, duration, or severity of angina or acute MI in patients with severe obstructive coronary artery disease
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety or effectiveness between older and younger subjects
- No studies in patients with renal impairment
- Not recommended in hepatically impaired patients
