Indications

  • Treatment of hypertension alone or with other antihypertensive agents
  • Initial therapy for patients likely to need multiple antihypertensive agents
  • Lowering blood pressure
  • Individualized blood pressure goals

Pharmacology

  • Amlodipine as dihydropyridine calcium channel blocker
  • Inhibition of transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
  • Greater effect on vascular smooth muscle cells than on cardiac muscle cells
  • Peripheral arterial vasodilator
  • Olmesartan Medoxomil as angiotensin II receptor antagonist
  • Blocking the vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Competitive inhibitor of the AT1 receptor
  • No inhibition of ACE

Dosage & Administration

  • Substitute individually titrated components
  • Initial therapy with 5/20 mg once daily
  • Titration up to a maximum of 10/40 mg once daily
  • Recommended starting dose of Amlodipine is 2.5 mg in patients 75 years
  • Not recommended for initial therapy in patients >75 years old

Interaction

  • Attenuation by NSAIDs including selective COX-2 inhibitors
  • Monitoring of blood pressure, renal function, and electrolytes in combination therapy

Contraindications

  • Cannot be co-administered with Aliskiren in patients with diabetes

  • Headache
  • Flushing
  • Dizziness
  • Sprue-like enteropathy

Pregnancy & Lactation

  • Pregnancy Category D
  • Avoid use in 2nd and 3rd trimester
  • Consideration of potential excretion in human milk

Precautions & Warnings

  • Use with caution in volume- or salt depleted patients
  • Risk of hypotension, vasodilation, angina, or acute MI in specific patient groups

Use in Special Populations

  • Lack of established safety and effectiveness in pediatric patients
  • No overall differences in safety or effectiveness in geriatric patients
  • No studies in patients with renal impairment
  • Not recommended for hepatically impaired patients
