Indications
- Treatment of severe extensive psoriasis
- Treatment of palmo-plantar pustular psoriasis
- Treatment of severe congenital ichthyosis
- Treatment of severe Darier’s disease
Pharmacology
- Acitretin is a retinoid
- Regulation of RNA/DNA synthesis
- Modulation of epidermal proliferation
- Modification of glycoprotein synthesis
- Modulation of immune response
- Cellular differentiation modulation in epidermis
Dosage & Administration
- Initial daily dose for adults and elderly: 25 mg or 30 mg for 2 to 4 weeks
- Daily dosage of 25-50 mg for 6 to 8 weeks
- Maximum daily dose of 75 mg
- Starting dose of 10 mg for patients with Darier’s disease
- Children dosage: about 0.5mg/kg
Interaction
- Concurrent intake with ethanol led to the formation of etretinate
- Contraindicated with methotrexate, tetracyclines, and vitamin A
- Reduction of protein binding of phenytoin
Contraindications
- Highly teratogenic in pregnant women
- Contraindicated in women of childbearing potential without strict contraception
- Contraindicated in severe hepatic or renal impairment
- Contraindicated in cases of hypersensitivity to the preparation
Side Effects
- Dose-related clinical side-effects are common
- Toxic dose is close to therapeutic dose
- Skin and mucous membranes are commonly affected
- Initial worsening of psoriasis symptoms may occur
Pregnancy & Lactation
- Contraindicated in pregnant women or nursing mothers
- Contraindicated for women who might become pregnant within 2 years of treatment cessation
Precautions & Warnings
- Exceptionally high risk of giving birth to a deformed child if taken before or during pregnancy
- Avoid excessive exposure to sunlight
- Regular monitoring of hepatic function, serum cholesterol and triglycerides
- Possible alteration of glucose tolerance
Overdose Effects
- Manifestations of acute Vitamin A toxicity
- Specific treatment unnecessary due to low acute toxicity
