Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gastric parietal cell

Dosage

  • Benign gastric and duodenal ulcer: 20 mg once daily for 4-8 weeks
  • NSAID-associated duodenal or gastric ulcer: 20 mg once daily for 4-8 weeks
  • Gastro-esophageal reflux disease: 20-40 mg once daily for 4-8 weeks
  • Long-term management of acid reflux disease: 10-20 mg daily
  • Acid-related dyspepsia: 10-20 mg once daily for 2-4 weeks
  • Prophylaxis of acid aspiration: 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery
  • Zollinger-Ellison syndrome: Usual range 20-120 mg daily
  • Helicobacter pylori eradication regimen: 20 mg twice daily in combination with antimicrobial agents
  • Pediatric use in severe ulcerating reflux esophagitis: 10-40 mg once daily for 4-12 weeks
  • IV Injection: Various doses for different indications

Administration

  • IV Injection for intravenous administration only
  • IV Infusion over a period of 20-30 minutes or more
  • Directions for reconstitution and slow administration
  • Restrictions on co-administration with other drugs
  • Use within specified time after reconstitution

Interaction

  • Effect on absorption of ketoconazole
  • Metabolism through cytochrome P450
  • Potential delay in elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations during concomitant administration with clarithromycin
  • No evidence of interaction with certain drugs
  • Effect on absorption of digoxin
  • Effect of increased intragastric pH on certain drugs

Contraindications

  • Known hypersensitivity to omeprazole
  • Need to exclude malignancy before treatment in suspected gastric ulcer cases

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme in isolated cases
  • Diarrhea, headache, constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dizziness, somnolence, insomnia, and vertigo
  • Mental confusion, agitation, depression, and hallucinations in severely ill patients
  • Arthritic and myalgic symptoms

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus/newborn child based on epidemiological studies
  • Use during pregnancy with no adverse effects observed
  • No information on passage into breast milk or effects on the neonate
  • Discontinuation of breast-feeding if essential use of omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Possible association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis in long-term treatment cases
  • Potential for methotrexate toxicities with concomitant use
