Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocker of hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 20 mg once daily for NSAID-associated duodenal or gastric ulcer
  • 20 mg once daily for gastro-esophageal reflux disease
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for acid-related dyspepsia
  • 40 mg on the preceding evening and 2-6 hours before surgery for prophylaxis of acid aspiration
  • Initially 60 mg once daily for Zollinger-Ellison syndrome
  • Omeprazole is recommended at a dose of 20 mg twice daily in association with antimicrobial agents for Helicobacter pylori eradication regimen
  • 10-20 mg once daily for severe ulcerating reflux esophagitis in pediatric use
  • 40 mg intravenously for prophylaxis of acid aspiration
  • 40 mg intravenously for duodenal ulcer, gastric ulcer, or reflux esophagitis
  • Initial intravenous dose of 60 mg daily for Zollinger-Ellison syndrome
  • IV infusion over 20-30 minutes or more
  • Oral and IV administration instructions

Administration

  • IV injection for intravenous administration only
  • IV administration for infusion over a period of 20-30 minutes or more
  • Reconstitution and administration instructions for IV injection and IV infusion

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolism through cytochrome P450 affecting the elimination of diazepam, phenytoin, and warfarin
  • Monitoring and potential dose reduction of warfarin or phenytoin
  • Increased plasma concentrations during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids
  • No effect on absorption by alcohol or food
  • No evidence of interaction with piroxicam, diclofenac, or naproxen
  • Increased bioavailability of digoxin during simultaneous treatment with Omeben

Contraindications

  • Known hypersensitivity to Omeprazole
  • Exclusion of malignancy before treatment when gastric ulcer is suspected

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia
  • Diarrhoea and headache
  • Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dry mouth, stomatitis, and candidiasis
  • Paraesthesia, dizziness, light headedness, and feeling faint
  • Somnolence, insomnia, and vertigo
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral oedema, increased sweating, gynaecomastia, and hematological changes
  • Hepatic effects including hepatitis, interstitial nephritis, and hepatic failure
  • Increases in liver enzymes

Pregnancy & Lactation

  • No adverse effects on pregnancy or on the health of the fetus/newborn child based on epidemiological studies
  • Safe to use during pregnancy
  • Discontinuation of breastfeeding if essential use of Omeprazole is considered
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Potential association with increased risk for osteoporosis-related fractures
  • Occasional atrophic gastritis in long-term treatment
  • Potential for methotrexate toxicities with concomitant use of PPIs
