Indications
- Gastric and duodenal ulcer

Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of the antisecretory effect occurs within one hour
- Maximum effect occurs within two hours
- Inhibition of secretion lasts up to 72 hours
- Secretory activity returns gradually over 3 to 5 days after discontinuation

Administration
- Direction for use of IV Injection

Interaction
- Decreased intragastric acidity affecting the absorption of other drugs
- Metabolism through cytochrome P450 delaying the elimination of other drugs
- Recommendation to monitor patients receiving warfarin or phenytoin
- Plasma concentrations of Omeprazole and clarithromycin increased during concomitant administration
- No evidence of interaction with specific drugs (phenacetin, theophylline, caffeine, etc.)
- Simultaneous treatment with digoxin leads to a 10% increase in the bioavailability of digoxin
Contraindications
- Contraindicated in patients with known hypersensitivity to omeprazole
- Exclusion of malignancy in suspected cases of gastric ulcer before treatment initiation
Side Effects
- Well tolerated with generally mild and reversible adverse reactions
- Reported skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme reported in isolated cases
- Diarrhea and headache reported
- Gastrointestinal reactions like constipation, nausea/vomiting, flatulence, and abdominal pain
- Dizziness, light-headedness, and feeling faint associated with treatment
- Reversible mental confusion, agitation, depression, and hallucinations reported predominantly in severely ill patients
- Arthritic and myalgic symptoms reported
Pregnancy & Lactation
- No adverse effects on pregnancy or the health of the fetus/newborn child in prospective epidemiological studies
- Omeprazole can be used during pregnancy
- No information available on the passage of Omeprazole into breast milk or its effects on the neonate
- Discontinuation of breastfeeding if the use of Omeprazole is considered essential
- Safety and effectiveness not established in pediatric patients less than 18 years of age
Precautions & Warnings
- Avoid concomitant use of clopidogrel and omeprazole
- Observational studies suggest increased risk for osteoporosis-related fractures with PPI therapy
- Atrophic gastritis noted occasionally in gastric corpus biopsies from long-term treated patients
- Concomitant use with methotrexate may lead to methotrexate toxicities
