Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days after discontinuation

Interaction

  • Effect on the absorption of ketoconazole
  • Metabolism in the liver through cytochrome P450
  • Impact on the elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin
  • No change in blood concentration of phenytoin in patients on continuous treatment with phenytoin during concomitant treatment
  • No change in coagulation time in patients on continuous treatment with warfarin during concomitant treatment
  • Plasma concentrations of Gastril and clarithromycin during concomitant administration
  • Absence of interaction with various drugs and food
  • Increased bioavailability of digoxin during simultaneous treatment with Gastril and digoxin

Contraindications

  • Known hypersensitivity to omeprazole
  • Suspicion of gastric ulcer and possibility of malignancy

Side Effects

  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme
  • Diarrhea and headache
  • Gastrointestinal reactions like constipation, nausea/vomiting, flatulence, and abdominal pain
  • Other reactions including dry mouth, stomatitis, candidiasis, paraesthesia, dizziness, and somnolence
  • Reversible mental confusion, agitation, depression, and hallucinations
  • Arthritic and myalgic symptoms
  • Rare reactions like blurred vision, taste disturbance, peripheral edema, and increased sweating
  • Reports of hepatic failure, increases in liver enzymes, and interstitial nephritis

Pregnancy & Lactation

  • No adverse effects of omeprazole on pregnancy or on the health of the fetus/newborn child based on epidemiological studies
  • Use of omeprazole in pregnancy
  • No information available on the passage of omeprazole into breast milk or its effects on the neonate

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Gastril
  • Possible association of PPI therapy with increased risk for osteoporosis-related fractures
  • Noted cases of atrophic gastritis in patients treated long-term with Gastril
  • Potential for methotrexate toxicities when used concomitantly with PPIs
