Indications

  • Treatment of gastric and duodenal ulcer
  • Treatment of NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Treatment of gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Treatment of acid-related dyspepsia
  • Treatment of severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Treatment of Zollinger-Ellison syndrome
  • Treatment of Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasting up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 40 mg daily for severe or recurrent cases of duodenal or gastric ulcer
  • 10-20 mg daily for long-term management of acid reflux disease
  • 1 g and Clarithromycin 500 mg both twice a day for one week for H. pylori eradication
  • IV Injection: Omeprazole 40 mg for prophylaxis of acid aspiration
  • IV Injection: Omeprazole IV 40 mg once daily for duodenal ulcer, gastric ulcer or reflux esophagitis
  • IV Injection: 60 mg daily for Zollinger-Ellison syndrome
  • Pediatric use: 10-40 mg once daily based on body-weight for severe ulcerating reflux esophagitis

Administration

  • IV Injection for intravenous administration only
  • IV Injection to be given slowly over a period of at least 2 to 5 minutes
  • IV Infusion over a period of 20-30 minutes
  • Reconstituted solution should be used within 4-12 hours based on the diluent used

Interaction

  • Decreased intragastric acidity affecting absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Delayed elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations of Omeprazole and clarithromycin increased during concomitant administration
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, and more

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer with possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, pruritus, and photosensitivity reported
  • Diarrhoea, headache, constipation, nausea/vomiting, and abdominal pain reported
  • Dizziness, light headedness, and feeling faint associated with treatment
  • Reversible mental confusion, agitation, depression, and hallucinations reported
  • Arthritic and myalgic symptoms reported

Pregnancy & Lactation

  • No adverse effects on pregnancy or health of the fetus/newborn child observed
  • Use during pregnancy can be considered
  • No information available on passage into breast milk or effects on the neonate
  • Discontinuation of breastfeeding if essential to use Omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Possibly associated with increased risk for osteoporosis-related fractures
  • Atrophic gastritis occasionally noted in gastric corpus biopsies from long-term treatment
