Indications
- Treatment of gastric and duodenal ulcer
- Treatment of NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Treatment of gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Treatment of acid-related dyspepsia
- Treatment of severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Treatment of Zollinger-Ellison syndrome
- Treatment of Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasting up to 72 hours
- Gradual return of secretory activity over 3 to 5 days after discontinuation
Dosage
- 20 mg once daily for benign gastric and duodenal ulcer
- 40 mg daily for severe or recurrent cases of duodenal or gastric ulcer
- 10-20 mg daily for long-term management of acid reflux disease
- 1 g and Clarithromycin 500 mg both twice a day for one week for H. pylori eradication
- IV Injection: Omeprazole 40 mg for prophylaxis of acid aspiration
- IV Injection: Omeprazole IV 40 mg once daily for duodenal ulcer, gastric ulcer or reflux esophagitis
- IV Injection: 60 mg daily for Zollinger-Ellison syndrome
- Pediatric use: 10-40 mg once daily based on body-weight for severe ulcerating reflux esophagitis
Administration
- IV Injection for intravenous administration only
- IV Injection to be given slowly over a period of at least 2 to 5 minutes
- IV Infusion over a period of 20-30 minutes
- Reconstituted solution should be used within 4-12 hours based on the diluent used
Interaction
- Decreased intragastric acidity affecting absorption of ketoconazole
- Metabolized in the liver through cytochrome P450
- Delayed elimination of diazepam, phenytoin, and warfarin
- Plasma concentrations of Omeprazole and clarithromycin increased during concomitant administration
- No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, and more
Contraindications
- Known hypersensitivity to Omeprazole
- Suspicion of gastric ulcer with possibility of malignancy
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, pruritus, and photosensitivity reported
- Diarrhoea, headache, constipation, nausea/vomiting, and abdominal pain reported
- Dizziness, light headedness, and feeling faint associated with treatment
- Reversible mental confusion, agitation, depression, and hallucinations reported
- Arthritic and myalgic symptoms reported
Pregnancy & Lactation
- No adverse effects on pregnancy or health of the fetus/newborn child observed
- Use during pregnancy can be considered
- No information available on passage into breast milk or effects on the neonate
- Discontinuation of breastfeeding if essential to use Omeprazole
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Possibly associated with increased risk for osteoporosis-related fractures
- Atrophic gastritis occasionally noted in gastric corpus biopsies from long-term treatment
