Indications

  • Treatment of gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H/K ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Return of secretory activity over 3 to 5 days after discontinuation

Dosage

  • Oral dosage for various conditions
  • Maintenance dose for recurrent duodenal ulcer
  • IV Injection dosage for different conditions
  • Paediatric use dosage in severe ulcerating reflux esophagitis

Administration

  • Direction for use of IV Injection

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolism through cytochrome P450 affecting diazepam, phenytoin, and warfarin elimination
  • Plasma concentrations of Omeprazole and clarithromycin are increased during concomitant administration
  • No evidence of interaction with phenacetin, theophylline, caffeine, or antacids
  • Absorption of Omeprazole not affected by alcohol or food

Contraindications

  • Known hypersensitivity to Omeprazole
  • Suspicion of gastric ulcer should exclude malignancy before treatment

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Diarrhoea and headache
  • Gastrointestinal reactions including constipation, nausea/vomiting, and abdominal pain
  • Dizziness, light headedness, and feeling faint
  • Mental confusion, agitation, depression, and hallucinations

Pregnancy & Lactation

  • No adverse effects of Omeprazole on pregnancy or fetus/newborn child
  • No information available on passage into breast milk
  • Discontinue breastfeeding if essential use of Omeprazole

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and Protobit
  • Potential association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in gastric corpus biopsies from long-term use
  • Concomitant use with methotrexate may lead to toxicities
