Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- As prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasts up to 72 hours
- Discontinuation leads to gradual return of secretory activity over 3 to 5 days
Dosage
- Oral administration for specific conditions and durations
- IV injection for prophylaxis of acid aspiration
- IV infusion for duodenal ulcer, gastric ulcer, or reflux oesophagitis
- Paediatric use in severe ulcerating reflux esophagitis
Administration
- Direction for use of IV Injection
- Direction for use of IV Infusion
- Intravenous administration only
- Slow intravenous injection
- Intravenous infusion over a period of 20-30 minutes
Interaction
- Decreased intragastric acidity affecting absorption of ketoconazole
- Metabolism through cytochrome P450 affecting diazepam, phenytoin, and warfarin
- Plasma concentrations of Perizol and clarithromycin are increased during concomitant administration
- No evidence of interaction with specific drugs, alcohol, or food
- Simultaneous treatment with digoxin leads to a 10% increase in the bioavailability of digoxin
Contraindications
- Hypersensitivity to Omeprazole
- Possibility of malignancy when gastric ulcer is suspected
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhoea, headache, constipation, nausea/vomiting, and abdominal pain
- Dry mouth, stomatitis, and candidiasis
- Dizziness, light headedness, and feeling faint
- Mental confusion, agitation, and depression
- Arthritic and myalgic symptoms
- Other isolated cases of adverse reactions
Pregnancy & Lactation
- No adverse effects on pregnancy or fetus/newborn child from prospective epidemiological studies
- Use during pregnancy with no adverse effects indicated
- No information available on passage into breast milk or effects on the neonate
- Discontinuation of breastfeeding if essential use is considered
Precautions & Warnings
- Avoid concomitant use of clopidogrel and Perizol
- Possible increased risk for osteoporosis-related fractures with PPI therapy
- Atrophic gastritis noted occasionally in long-term use
- Concomitant use with methotrexate may lead to toxicities
