Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer

Pharmacology
Inhibitor of gastric acid secretion by blocking hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell. Onset of antisecretory effect occurs within one hour, with the maximum effect within two hours and inhibition of secretion lasts up to 72 hours.
Dosage
- 20 mg once daily for 4-8 weeks in gastric ulceration
- 10-40 mg daily for various conditions
- Recommended dose in various gastric conditions
- Different doses for pediatric use
- 40 mg IV injection for prophylaxis of acid aspiration
- 60 mg IV injection for Zollinger-Ellison syndrome
- 20 mg twice daily for Helicobacter pylori eradication regimen

Interaction
Interactions with ketoconazole, diazepam, phenytoin, warfarin, clarithromycin, phenacetin, theophylline, caffeine, and other drugs. No interactions with alcohol or certain NSAIDs
Contraindications
Contraindicated in patients with known hypersensitivity to omeprazole. Possibility of malignancy should be excluded before treatment.
Side Effects
- Skin rash, urticaria, and pruritus
- Photosensitivity, bullous eruption, and erythema multiforme
- Diarrhea and headache
- Gastrointestinal reactions
- Dizziness, somnolence, and insomnia
- Mental confusion, agitation, and depression
- Arthritic and myalgic symptoms
- Blood and liver-related side effects
- Other isolated cases of side effects
Pregnancy & Lactation
No adverse effects of Omeprazole on pregnancy or fetus/newborn child. Use during pregnancy is acceptable. Discontinue breastfeeding if essential.
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- Potential association with increased risk for osteoporosis-related fractures
- Atrophic gastritis in long-term use
- Potential methotrexate toxicities with concomitant use of PPIs
