Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of effect within one hour
  • Maximum effect within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Gradual return of secretory activity over 3 to 5 days after discontinuation

  • IV injection dosage for prophylaxis of acid aspiration, duodenal ulcer, gastric ulcer, reflux esophagitis, and Zollinger-Ellison syndrome

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolism through cytochrome P450 affecting diazepam, phenytoin, and warfarin elimination
  • Monitoring of warfarin or phenytoin levels during concomitant treatment
  • Plasma concentration increase during concomitant administration of Stosec and clarithromycin
  • No evidence of interaction with various drugs like phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, antacids, piroxicam, diclofenac, naproxen, alcohol, or food

Contraindications

  • Known hypersensitivity to Omeprazole
  • Exclusion of malignancy possibility in suspected gastric ulcer before treatment

Side Effects

  • Mild and reversible adverse reactions
  • Reported symptoms including skin rash, urticaria, pruritus, photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia, diarrhea, headache, constipation, nausea/vomiting, flatulence, abdominal pain, dry mouth, stomatitis, candidiasis, and more

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus based on epidemiological studies
  • Use during pregnancy possible
  • No information on passage into breast milk or effects on neonate
  • Discontinuation of breastfeeding if essential use of Omeprazole

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Potential association with increased risk for osteoporosis-related fractures
  • Occasional atrophic gastritis noted in long-term treatment
  • Potential for methotrexate toxicities with concomitant use
