Indications

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocker of hydrogen-potassium-adenosine triphosphatase enzyme system

Dosage

  • Benign gastric and duodenal ulcer - 20mg once daily for 4 weeks in duodenal ulceration, 8 weeks in gastric ulceration
  • NSAID-associated duodenal or gastric ulcer - 20mg once daily for 4 weeks
  • Gastro-esophageal reflux disease - 20mg once daily for 4 weeks, continued for further 4-8 weeks if not fully healed
  • Long-term management of acid reflux disease - 10-20mg daily
  • Prophylaxis of acid aspiration - 40mg on the preceding evening, then 40mg 2-6 hours before surgery
  • Zollinger-Ellison syndrome - Initially 60mg once daily
  • Helicobacter pylori eradication regimen in peptic ulcer disease - 20mg twice daily in association with antimicrobial agents

Administration

  • Intravenous administration only
  • Slow intravenous injection
  • Intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolism in the liver through cytochrome P450 affecting diazepam, phenytoin, and warfarin
  • Plasma concentrations increased during concomitant administration with clarithromycin
  • No evidence of interaction with phenacetin, theophylline, caffeine, propranolol, and more

Contraindications

  • Known hypersensitivity
  • Suspicion of gastric ulcer with possibility of malignancy

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, pruritus
  • Diarrhoea, headache, constipation
  • Dry mouth, stomatitis, candidiasis
  • Dizziness, somnolence, reversible mental confusion
  • Arthritic and myalgic symptoms
  • Blurred vision, taste disturbance, peripheral oedema

Pregnancy & Lactation

  • No adverse effects on pregnancy or fetus/child health
  • Use during pregnancy is acceptable
  • No information on passage into breast milk, discontinuation of breastfeeding if essential

Precautions & Warnings

  • Avoid concomitant use with clopidogrel
  • Possible association with increased risk for osteoporosis-related fractures
  • Occasional atrophic gastritis in long-term treatment
  • Concomitant use with methotrexate may lead to toxicities
