Indications
- Gastric and duodenal ulcer
- NSAID-associated duodenal and gastric ulcer
- As prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcer
- Gastro-esophageal reflux disease
- Long-term management of acid reflux disease
- Acid-related dyspepsia
- Severe ulcerating reflux esophagitis
- Prophylaxis of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Helicobacter pylori-induced peptic ulcer
Pharmacology
Inhibitor of gastric acid secretion; blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in the gastric parietal cell; onset of antisecretory effect occurs within one hour, with maximum effect within two hours; inhibition of secretion lasts up to 72 hours; gradually returns to normal after discontinuation over 3 to 5 days
Dosage
- Oral dosage for different conditions
- IV Injection dosage for prophylaxis of acid aspiration, duodenal ulcer, gastric ulcer, reflux esophagitis, Zollinger-Ellison syndrome
Administration
- IV Injection: Intravenous administration only; should be given as a slow intravenous injection; solution should be used within 4 hours of reconstitution
- IV Infusion: Intravenous infusion over a period of 20-30 minutes or more; reconstituted solution should not be mixed with any other drug
Interaction
May affect the absorption and elimination of other medications; monitoring recommended with concurrent warfarin or phenytoin treatment; increased plasma concentrations during coadministration with certain medications
Contraindications
Contraindicated in patients with known hypersensitivity; possibility of malignancy should be excluded before treatment
Side Effects
- Mild and reversible adverse reactions reported; skin rash, urticaria, pruritus, photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia, diarrhea, headache, constipation, nausea/vomiting, flatulence, dry mouth, stomatitis, candidiasis, paraesthesia, dizziness, mental confusion, depression, arthritic and myalgic symptoms, blurred vision, taste disturbance, peripheral edema, increased sweating, and more
Pregnancy & Lactation
No adverse effects on pregnancy or fetus reported in studies; use during pregnancy is acceptable; effects on breast milk and newborn unknown
Precautions & Warnings
- Avoid concomitant use with clopidogrel
- May be associated with increased risk for osteoporosis-related fractures
- Atrophic gastritis noted in long-term treatment
- Concomitant use with methotrexate may lead to toxicities
