Indications

  • Gastric and duodenal ulcer

Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect occurs within one hour
  • Maximum effect occurs within two hours
  • Inhibition of secretion lasts up to 72 hours
  • Secretory activity returns gradually over 3 to 5 days after discontinuation

Dosage

  • Oral administration for various conditions and durations
  • IV Injection for prophylaxis of acid aspiration, duodenal ulcer, gastric ulcer, or reflux oesophagitis
  • Pediatric use in severe ulcerating reflux esophagitis

Administration

  • IV Injection: slow intravenous administration
  • IV Infusion: intravenous infusion over a period of 20-30 minutes or more

Interaction

  • Decreased intragastric acidity affecting ketoconazole absorption
  • Metabolism through cytochrome P450 affecting elimination of diazepam, phenytoin and warfarin
  • Plasma concentrations interaction with clarithromycin during H. pylori eradication
  • No interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids
  • Increased bioavailability of digoxin during simultaneous treatment

Contraindications

  • Known hypersensitivity to omeprazole
  • Need to exclude malignancy before treatment if gastric ulcer is suspected

Side Effects

  • Mild and reversible adverse reactions reported
  • Skin rash, urticaria, pruritus, photosensitivity, bullous eruption, and more
  • Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain
  • Dizziness, light headedness, somnolence, insomnia, vertigo, mental confusion, agitation, and more
  • Arthritic and myalgic symptoms, blurred vision, taste disturbance, and more
  • Increases in liver enzymes observed

Pregnancy & Lactation

  • No adverse effects on pregnancy or health of fetus/newborn child based on studies
  • Use during pregnancy considered safe

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Possible association of PPI therapy with increased risk of osteoporosis-related fractures

  • Potential for methotrexate toxicities with concomitant PPI use
