Indications

  • Gastric and duodenal ulcer
  • NSAID-associated duodenal and gastric ulcer
  • Gastro-esophageal reflux disease
  • Acid-related dyspepsia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer
  • Long-term management of acid reflux disease
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia

Pharmacology

Inhibitor of gastric acid secretion, effect occurs within one hour, maximum effect within two hours, inhibition of secretion lasts up to 72 hours

Administration

  • IV Injection for intravenous administration only

Interaction

  • Decreased intragastric acidity affecting absorption of ketoconazole
  • Metabolism through cytochrome P450 affecting elimination of diazepam, phenytoin, and warfarin
  • Plasma concentrations and interactions with clarithromycin, phenytoin, warfarin, ketoconazole, digoxin
  • No interactions with various drugs and food

Contraindications

Known hypersensitivity, suspicion of malignancy for gastric ulcer

Side Effects

  • Skin rash, urticaria, pruritus
  • Photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia
  • Diarrhea, headache, constipation, nausea/vomiting, flatulence, abdominal pain
  • Dry mouth, stomatitis, candidiasis
  • Paraesthesia, dizziness, light headedness, feeling faint
  • Somnolence, insomnia, vertigo
  • Mental confusion, agitation, depression, hallucinations
  • Arthritic and myalgic symptoms, blurred vision, taste disturbance
  • Peripheral edema, increased sweating, adverse hematological effects, hepatic and renal effects

Pregnancy & Lactation

Safe use during pregnancy, no information on breastfeeding, not established in pediatric patients

Precautions & Warnings

  • Avoid concomitant use of clopidogrel
  • Association with increased risk of osteoporosis-related fractures
  • Occasional atrophic gastritis in long-term use
  • Potential methotrexate toxicities with PPIs
