Indications
- Prevention of nausea and vomiting associated with initial and repeat courses of emetogenic cancer chemotherapy

Description
- Orally dissolving film designed to be applied on top of the tongue
- Dissolves within 20 seconds and then is swallowed with saliva
- Active ingredient: Ofmit base, the racemic form of Ofmit, and a selective blocking agent
- Molecular weight: 293.3
Pharmacology
- Potent, highly selective 5HT3 receptor-antagonist
- Mode of action not fully known
- Antagonizes 5HT3 receptors in the peripheral and central nervous systems
- Precise mode of action in post-operative nausea and vomiting not known
Dosage
- Specific dosages for chemotherapy-induced nausea and vomiting, radiotherapy-induced nausea and vomiting, and postoperative nausea and vomiting
- Different dosages for adults, pediatric patients, and geriatric patients
- Oral solution for radiotherapy-induced nausea and vomiting and postoperative nausea and vomiting
- Oral soluble film for prevention of nausea and vomiting associated with highly emetogenic cancer chemotherapy and moderately emetogenic cancer chemotherapy
Administration
- Instructions for tearing the pouch, placing the film on the tongue, and swallowing it after dissolution
- No requirement for water during administration
Interaction
- Metabolism by hepatic cytochrome P-450 drug-metabolizing enzymes
- Potential for clearance and half-life change with inducers or inhibitors of these enzymes
Contraindications
- Hypersensitivity to the drug or any of its components
- Concomitant use of apomorphine
Side Effects
- Frequently reported adverse events
- Majority of adverse events are mild or moderate in nature
- Rare cases of anaphylaxis, brochospasm, tachycardia, chest pain, hypokalemia, shortness of breath reported
- No evidence of extrapyramidal reactions
Pregnancy & Lactation
- No evidence of impaired fertility or harm to the fetus in animal studies
- Caution advised in nursing women
Precautions & Warnings
- Hypersensitivity reactions reported in patients with hypersensitivity to other selective 5-HT3 receptor antagonists
- Not a drug that stimulates gastric or intestinal peristalsis
- Caution advised in patients following abdominal surgery or in patients with chemotherapy-induced nausea and vomiting
Use in Special Populations
- Same dosage for patients with impaired renal function
- Dosage adjustment for patients with severe hepatic impairment
- Limited information about dosage in pediatric patients 4 years of age or younger
- No dosage adjustment needed in patients over the age of 65
