Indications
- Treatment of obese patients with BMI > 30 kg/m^2
- Treatment of overweight patients with BMI > 28 kg/m^2 and associated risk factors
- Treatment of adolescents (12 years & older) with obesity and complications
Pharmacology
- Potent and long-acting lipase inhibitor
- Forms covalent bond with active serine site of gastric and pancreatic lipases
- Inactivated enzyme unable to hydrolyze dietary fats
- Caloric deficit effect on weight control
- Systemic absorption not needed for activity
Dosage & Administration
- Recommended dose of 120 mg capsule
- To be taken immediately before, during, or up to one hour after each main meal
- Increase in fecal fat 24-48 hours after dosing
- Safety & efficacy studied in clinical trials lasting up to 4 years
Interaction
- Reduction in cyclosporine plasma levels when coadministered
- Inhibition of absorption of vitamin E acetate supplement
- Effects on absorption of vitamin D, vitamin A, and vitamin K not known
- Possible hypothyroidism with concomitant use of levothyroxine
- Possible decreased vitamin K absorption in patients on warfarin
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Hypersensitivity to orlistat or ingredients of the capsules
Side Effects
- Common gastrointestinal side effects such as oily spotting, flatulence
- Rare cases of influenza, anxiety, headache, fatigue
- Rare cases of hypersensitivity with clinical symptoms
Pregnancy & Lactation
- Pregnancy Category X drug
- Contraindicated during pregnancy
- Caution required during lactation
Precautions & Warnings
- Organic causes of obesity should be excluded before prescribing
- Avoid coadministration with cyclosporine
- Close monitoring of antidiabetic drug treatment during therapy
- Adherence to dietary recommendations and fat intake distribution
- Encouragement of multivitamin supplement intake
Overdose Effects
- Studied single doses of 800 mg and multiple doses of up to 400 mg three times a day
- No significant adverse findings reported
