Indications
- Monotherapy in the treatment of partial seizures for adults
- Adjunctive therapy in the treatment of partial seizures for adults
- Monotherapy in the treatment of partial seizures in children aged 4-16 years
- Adjunctive therapy in the treatment of partial seizures in children aged 2-16 years
Pharmacology
- Primary pharmacological activity exerted through the metabolite derivative (MHD) of Oxcarbazepine
- Mechanism of action based on blockade of voltage-sensitive sodium channels
- Stabilization of hyper excited neural membranes
- Inhibition of repetitive neuronal firing
- Diminishment of propagation of synaptic impulses
Dosage & Administration
- Recommended initial dose for adults: 600 mg/day, given twice-a-day
- Maximum weekly increment for adjunctive therapy in adults: 600 mg/day
- Recommended daily dose for adults: 1200 mg/day for adjunctive therapy
- Recommended maximum daily dose for adults on monotherapy: 2400 mg/day
- Recommended increment for initiation of monotherapy in adults: 300 mg/day every third day
- Dosage adjustment for patients with creatinine clearance <30 mL/min
- Initial dose for pediatrics: 8 to 10 mg/kg/day, given twice-a-day
- Consideration of higher starting dose for pediatric patients aged 2 to <4 years
- Target maintenance dose achievement over specific periods for pediatric patients
- Increment for initiation of monotherapy in pediatric patients: 5 mg/kg/day every third day
- Maximum weekly increment for conversion to monotherapy in pediatric patients: 10 mg/kg/day
- Withdrawal timeframe for concomitant antiepileptic drugs during conversion to monotherapy
Interaction
- Inhibition of CYP2C19 enzyme by Oxcarbazepine and its metabolite
- Possible interactions with medicinal products metabolized by CYP2C19
- Potential impact on hormonal contraceptives
- Lowering of AUC of felodipine and bioavailability of MHD by co-administration with verapamil
- Concurrent use with phenobarbital and phenytoin
Contraindications
- Hypersensitivity to the active substance or any of the excipients
Side Effects
- Commonly reported adverse reactions: somnolence, headache, dizziness, diplopia, nausea, vomiting, fatigue
- Rare development of clinically significant hyponatremia
- Occurrence of Class I (immediate) hypersensitivity reactions
- Reports of rash, pruritus, urticaria, angioedema, and anaphylaxis
Pregnancy & Lactation
- Potential for causing serious birth defects when administered during pregnancy
- Recommendation for administration of vitamin K1 as a preventive measure
- Excretion of Oxcarbazepine and its active metabolite in human breast milk
- Inhibition of Oxazep use during breastfeeding
Precautions & Warnings
- Caution regarding the combination of alcohol with Oxazep therapy
- Necessity for gradual withdrawal to minimize potential increase in seizure frequency
