Indications
- Monotherapy in adults for partial seizures
- Adjunctive therapy in adults for partial seizures
- Monotherapy in children 4-16 years for partial seizures
- Adjunctive therapy in children 2-16 years for partial seizures
Pharmacology
- Mechanism of action is based on blockade of voltage-sensitive sodium channels
- Exerts activity primarily through the metabolite derivative (MHD) of Oxcarbazepine
Dosage & Administration
- Adults initiate with a dose of 600 mg/day, given twice-a-day
- Maximum daily dose for adults is 2400 mg/day
- Pediatrics initiate with 8-10 mg/kg/day, given twice-a-day
- Maximum maintenance dose for pediatrics should not exceed 60 mg/kg/day
Interaction
- Inhibits the enzyme CYP2C19
- Interactions with CYP2C19-metabolized medicinal products
- May render some hormonal contraceptives ineffective
- May lower AUC of felodipine and decrease bioavailability of MHD with verapamil
Contraindications
- Hypersensitivity to the active substance or any excipients
Side Effects
- Most commonly reported adverse reactions: somnolence, headache, dizziness, diplopia, nausea, vomiting, fatigue
- Rarely clinically significant hyponatremia may develop
- Reports of class I hypersensitivity reactions including rash, pruritus, urticaria, angioedema, and anaphylaxis
Pregnancy & Lactation
- May cause serious birth defects when administered during pregnancy
- May lead to bleeding disorders in the newborn
- Oxcarbazepine and its metabolite cross the placenta
- Excreted in human breast milk, should not be used during breast-feeding
Precautions & Warnings
- Caution should be exercised if alcohol is taken in combination with therapy
- Should be withdrawn gradually to minimize potential of increased seizure frequency
