Indications

  • Adjunct to diet and exercise for type 2 diabetes mellitus
  • Reduction of cardiovascular death risk in type 2 diabetes patients with established cardiovascular disease

Pharmacology

Sodium glucose co-transporter-2 (SGLT-2) inhibitor, responsible for reabsorption of glucose from kidney's glomerular filtrate, leading to reduced renal absorption and lower renal threshold for glucose

Dosage & Administration

  • Recommended dose of 10 mg once daily
  • Can be taken with or without food
  • Dose may be increased to 25 mg once daily in tolerant patients
  • Correct volume depletion prior to initiation

Interactions

  • Co-administration with diuretics increases urine volume
  • Increased risk of hypoglycemia when co-administered with insulin or insulin secretagogues
  • SGLT2 inhibitors lead to positive urine glucose tests, alternative monitoring methods needed

Contraindications

  • History of serious hypersensitivity reaction to the product or its ingredients
  • Severe renal impairment
  • End-stage renal disease
  • Dialysis

  • Dehydration
  • Hypotension
  • Weakness
  • Dizziness
  • Increased thirstiness

Pregnancy & Lactation

  • Use during pregnancy only if potential benefit justifies potential risk to fetus
  • Not recommended during breastfeeding

Precautions & Warnings

  • Assessment of renal function recommended prior to initiation
  • No initiation in patients with eGFR less than 45 ml/min/1.73 m^2

Overdose Effects

Employ usual supportive measures in case of overdose, hemodialysis efficacy not studied
