Indications
- Reduces rate of thrombotic cardiovascular events
- Management of ACS in PCI patients
Pharmacology
- Platelet activation and aggregation inhibitor
Dosage & Administration
- Initiate with 60 mg oral loading dose
- Continue with 10 mg once daily
- Consider 5 mg once daily for patients <60 kg
- Take with or without food
- Co-administration with aspirin daily
Interaction
- Increased bleeding risk with warfarin
- Increased bleeding risk with NSAIDs
- Can be administered with specific drugs including aspirin, heparin, GPIIb/IIIa inhibitors, statins, drugs that elevate gastric pH
Contraindications
- Active pathological bleeding
- History of TIA or stroke
Side Effects
- Bleeding
- Thrombotic thrombocytopenic purpura
- Other side effects (headache, back pain, dyspnea, nausea, hypertension, bradycardia, rash etc)
Pregnancy & Lactation
- No well-controlled studies in pregnant women
- Unknown excretion in human milk
- Use during nursing only if potential benefit justifies potential risk to infant
Precautions & Warnings
- Increased bleeding risk in CABG patients
- Risk of stent thrombosis, MI, and death with premature discontinuation
Use in Special Populations
- No established safety and effectiveness in pediatric patients
- Increased risk of fatal bleeding events in geriatric patients
- No dosage adjustment for renal impairment
- No dosage adjustment for mild to moderate hepatic impairment
Overdose Effects
- Lethality observed in rats at high doses
- Platelet transfusion may restore clotting ability
- Active metabolite not likely to be removed by dialysis
