Indications
- Reduce rate of thrombotic cardiovascular events
- Patients with acute coronary syndrome
- Unstable angina or non-ST-elevation myocardial infarction
- ST-elevation myocardial infarction
- Managed with percutaneous coronary intervention
- Stent thrombosis
Pharmacology
- Inhibitor of platelet activation and aggregation
- Irreversible binding to P2Y12 class of ADP receptors
Dosage & Administration
- Oral loading dose of 60 mg
- Maintenance dose of 10 mg once daily
- Consider 5 mg in patients <60 kg
- Take with or without food
- Co-administration with daily aspirin
Interaction
- Increased risk of bleeding with warfarin
- Increased risk of bleeding with NSAIDs
- Can be administered with aspirin, heparin, GPIIb/IIIa inhibitors, statins, drugs that elevate gastric pH
Contraindications
- Active pathological bleeding
- History of transient ischemic attack or stroke
Side Effects
- Bleeding
- Thrombotic thrombocytopenic purpura
- Other side effects (headache, back pain, dyspnea, nausea, hypertension, bradycardia, rash)
Pregnancy & Lactation
- No adequate studies in pregnant women
- No evidence of fetal harm in animal studies
- Unknown excretion in human milk
Precautions & Warnings
- Increased risk of bleeding in CABG
- Risk of stent thrombosis, MI, and death upon discontinuation
Use in Special Populations
- Pediatric use not established
- Increased risk of fatal bleeding events in patients >75 years
- No dosage adjustment for renal impairment
- No dosage adjustment for mild to moderate hepatic impairment
Overdose Effects
- Lethality observed at high doses
- Platelet transfusion may restore clotting ability
- Dialysis may not remove active metabolite
