Description
- Pregavax is an anticonvulsant used for the management of neuropathic pain associated with diabetic peripheral neuropathy (DPN)
- It is also indicated for postherpetic neuralgia (PHN)
- It is an adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
- Additionally, it is used for the management of fibromyalgia and neuropathic pain associated with spinal cord injury
Dosage Information
- Maximum recommended dose of Pregabalin is 100 mg three times a day for DPN patients with creatinine clearance of at least 60 ml/min
- For PHN, recommended dose of Pregabalin is 75 to 150 mg two times a day or 50 to 100 mg three times a day
- Recommended dose for fibromyalgia is 300 to 450 mg/day
- For neuropathic pain associated with spinal cord injury, the recommended dose range is 150 to 600 mg/day
- Pregabalin CR tablet should be administered after an evening meal
- Dosing instructions for switching from Pregabalin capsules to Pregabalin CR capsule tablet are provided
Administration
- Pregabalin is taken orally, with or without food
- Pregabalin CR tablet should be swallowed whole after an evening meal, not split, crushed, or chewed
- Patients should follow specific dosing instructions if they miss a dose of Pregabalin CR
Mode of Action
- Pregabalin is a structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA)
- It binds with high affinity to the alpha2-delta site in central nervous system tissues, which may be involved in its anti-nociceptive and antiseizure effects
Side Effects
- Common side effects in adults include dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and difficulty with concentration/attention
- Most common side effects in pediatric patients for the treatment of partial onset seizures are increased weight and increased appetite
Pregnancy & Lactation
- There are no adequate and well-controlled studies with pregabalin in pregnant women
- Small amounts of pregabalin have been detected in the milk of lactating women
- Breastfeeding is not recommended during treatment with pregabalin due to the potential risk of tumorigenicity
Precautions & Warnings
- Angioedema and hypersensitivity reactions can occur and may require immediate discontinuation of Pregavax
- Antiepileptic drugs, including Pregavax, increase the risk of suicidal thoughts or behavior
- Respiratory depression may occur when used with concomitant CNS depressants or in the setting of underlying respiratory impairment
- Patients' ability to drive or operate machinery may be impaired
- Increased seizure frequency or other adverse reactions may occur if Pregavax is rapidly discontinued
Use in Special Populations
- Safety and effectiveness in pediatric patients have not been established for several indications
- The safety and effectiveness of Pregavax extended-release tablet in pediatric patients have not been established
Overdose Effects
- Signs and symptoms of overdose include reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, and heart block
- There is no specific antidote, and general supportive care is indicated
