Indications

  • Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
  • Postherpetic neuralgia (PHN)
  • Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
  • Fibromyalgia
  • Neuropathic pain associated with spinal cord injury

Pharmacology

  • Structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA)
  • Binds with high affinity to the alpha2-delta site
  • Mechanism of action not fully elucidated

Dosage

  • Maximum recommended dose of 100 mg three times a day for DPN
  • Recommended dose of 75 to 150 mg two times a day or 50 to 100 mg three times a day for PHN
  • Recommended dose of 300 to 450 mg/day for fibromyalgia
  • Recommended dose range of 150 to 600 mg/day for spinal cord injury
  • Dosing for pediatric patients
  • Conversion from Pregabalin capsules to Pregabalin CR capsule tablet
  • Pregabalin tablet total daily dose

Administration

  • Oral route of administration
  • Can be taken with or without food
  • Pregabalin CR tablet should be administered after an evening meal

Interaction

  • Unlikely to be involved in significant pharmacokinetic drug interactions
  • No significant food and other drug interactions

Contraindications

  • Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components

  • Dizziness
  • Somnolence
  • Dry mouth
  • Edema
  • Blurred vision
  • Weight gain
  • Difficulty with concentration/attention
  • Increased weight and increased appetite in pediatric patients

Pregnancy lactation

  • No adequate and well-controlled studies in pregnant women
  • Small amounts detected in the milk of lactating women
  • Breastfeeding not recommended

Precautions warnings

  • Risk of angioedema and associated respiratory compromise
  • Risk of hypersensitivity reactions
  • Increased risk of suicidal thoughts or behavior
  • Potential respiratory depression
  • Impairment of driving or operating machinery
  • Increased seizure frequency if rapidly discontinued
  • Potential for peripheral edema
  • Caution when coadministering with thiazolidinedione antidiabetic agents

Use in special populations

  • Safety and effectiveness not established in pediatric patients for several indications
  • Safety and effectiveness not established for patients below the age of 1 month

Overdose effects

  • Reduced consciousness
  • Depression/anxiety
  • Confusional state
  • Agitation and restlessness
  • Seizures
  • Heart block
  • No specific antidote
