Indications

  • Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
  • Postherpetic neuralgia (PHN)
  • Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
  • Fibromyalgia
  • Neuropathic pain associated with spinal cord injury

Pharmacology

  • Structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA)
  • Binds with high affinity to the alpha2-delta site in central nervous system tissues

Dosage

  • Maximum recommended dose of Pregabalin for DPN: 100 mg three times a day
  • Maximum recommended dose of Pregabalin for PHN: 75 to 150 mg two times a day or 50 to 100 mg three times a day
  • Recommended dose of Pregabalin for fibromyalgia: 300 to 450 mg/day
  • Recommended dose range of Pregabalin for neuropathic pain associated with spinal cord injury: 150 to 600 mg/day

Administration

  • Oral route of administration
  • Can be taken with or without food
  • Pregabalin CR tablet should be administered after an evening meal
  • Should be swallowed whole and should not be split, crushed or chewed

Interaction

  • Nepco is unlikely to be involved in significant pharmacokinetic drug interactions
  • No drug interaction with food and others

Contraindications

  • Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components

Side Effects

  • Common side effects in adults: dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, thinking abnormal
  • Common side effects in pediatric patients for the treatment of partial onset seizures: increased weight, increased appetite

Pregnancy & Lactation

  • No adequate and well-controlled studies with pregabalin in pregnant women
  • Small amounts of pregabalin have been detected in the milk of lactating women

Precautions & Warnings

  • Angioedema can occur and may be associated with life threatening respiratory compromise requiring emergency treatment
  • Discontinue immediately in case of angioedema or hypersensitivity reactions
  • Increase the risk of suicidal thoughts or behavior
  • May cause dizziness, somnolence and impair patients' ability to drive or operate machinery

Use in Special Populations

  • Safety and effectiveness have not been established for pediatric patients
  • Safety and effectiveness in pediatric patients below the age of 1 month have not been established
  • The safety and effectiveness of Nepco extended-release tablet in pediatric patients have not been established

Overdose Effects

  • Signs and symptoms of overdose: reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, heart block
  • No specific antidote, general supportive care of the patient is indicated

Therapeutic Class

  • Adjunct anti-epileptic drugs
  • Primary anti-epileptic drugs
