Description
Lactu is a hyperosmolar laxative used for effective treatment of constipation. It is a non-absorbable disaccharide which draws water into the bowel, causing distention through fluid accumulation, promoting soft stool and accelerating bowel motion.
Indications
- Treatment of chronic constipation
- Relief from intestinal flora disturbances
- Reduction of increased blood ammonia levels in hepatopathy and portal-systemic encephalopathy
Dosage & Administration
- Constipation (Chronic Constipation):
- Adults: 3-6 tea-spoons initially, 1½-6 tea-spoons in long-term therapy
- Children up to 14 years: 3 tea-spoons initially, 1-2 tea-spoons in long-term therapy
- Infants and toddlers: 1-2 tea-spoons initially, 1 tea-spoon in long-term therapy
- Damaged Intestinal Flora:
- Adults: 1-2 tea-spoons daily
- Children: 1 tea-spoon daily
- Reduction of Blood Ammonia Level:
- Hyperammoniemia in Hepatopathy: 18-30 tea-spoons daily
- Portal Systemic Encephalopathy: 6-9 tea-spoons hourly for rapid laxation, with subsequent dose reduction
Mode of Action
Lactulose is metabolized in the colon by saccharolytic bacteria, producing low molecular weight organic acids which lower the pH of the colon contents, promote water retention by osmotic effect, and increase peristaltic activity.
Interaction
No significant drug interactions, but glycosidic effect of cardiac glycosides can be intensified by potassium deficiency in abuse.
Contraindications
- Hypersensitivity to galactose and lactose
- Galactose-free diet
- Gastro-cardial symptom complex
- Suspected intestinal obstruction
Side Effects
- Occasional flatulence, cramps, and abdominal discomfort at the beginning of treatment
- Rapidly eliminated by reducing the dose
- Overdose can result in diarrhea
- Loss of electrolytes, primarily potassium in abuse
Pregnancy & Lactation
US FDA Pregnancy Category B; no adverse effects observed. Use during pregnancy and lactation to be decided by a registered physician.
Precautions & Warnings
- Careful administration to patients intolerant to Lactu
- Consideration required for diabetics in (pre)coma hepaticum treatment
Overdose Effects
No reports of accidental overdosage. Expected symptoms include diarrhea and abdominal cramps.
