Pharmacology
- Phenothiazine derivative
- Blocks postsynaptic mesolimbic dopaminergic receptors
- Alpha-adrenergic blocking effect
- Reduces release of hypothalamic and hypophyseal hormones
- Competes with histamine for H1-receptor
- Muscarinic blocking effect
- Reduces stimuli to brainstem reticular system
- Well absorbed from gastrointestinal tract
- Clinical effects within 20 minutes after oral administration
- Metabolized by liver to various compounds
Dosage & Administration
- Initial dose for adults: 25 mg tablet at night
- Maximum adult dose: three 25 mg tablets at night
- Elderly: No specific dosage recommendations
- Children: Elixir containing 5 mg/5 ml
- Antihistamine dosage for children of 2-10 years
- Sedative dosage for children of 2-10 years
Interaction
- Enhances action of anticholinergic agents
- May interfere with immunologic urine pregnancy tests
- Should be discontinued before skin tests using allergen extracts
Contraindications
- Contraindicated in children less than two years old
- Hypersensitivity to promethazine or excipients
- Not for use in patients in coma or with CNS depression
- Avoid in patients who have taken monoamine oxidase inhibitors within previous 14 days

Pregnancy & Lactation
- Epidemiological evidence for safety in pregnancy
- Not recommended in two weeks prior to delivery
- Risks of prolonged neurological disturbances in infant if given in high doses during late pregnancy
- Excreted in breastmilk
- Risks of neonatal irritability and excitement
Precautions & Warnings
- Avoid in children and adolescents with signs suggestive of Reye’s syndrome
- May thicken or dry lung secretions
- Use with caution in patients with asthma, bronchitis, or bronchiectasis
- Use with care in patients with severe coronary artery disease, narrow angle glaucoma, epilepsy, hepatic or renal insufficiency
- Caution in patients with bladder neck obstruction or pyloro-duodenal obstruction
Overdose Effects
- Symptoms of severe overdose
- Variable symptoms in children
- Drowsiness or coma in adults
- Possible convulsions in both adults and children
- Tachycardia
- Cardiorespiratory depression uncommon
- Inducing vomiting with ipecacuanha
- Gastric lavage may be used
