Description

Mylac is a hyperosmolar laxative used to treat constipation. It is a non-absorbable disaccharide that draws water into the bowel, causing distention through fluid accumulation, promoting soft stool and accelerating bowel motion.

Pharmacology

Lactulose is a synthetic disaccharide metabolized in the colon by saccharolytic bacteria, producing low molecular weight organic acids. This lowers the pH of colon contents, promotes water retention, and increases peristaltic activity.

DosageAndAdministration

  • Constipation:
    • Adults:
      • 3-6 teaspoons daily initially
      • 1.5-6 teaspoons daily in long-term therapy
    • Children (up to 14 years):
      • 3 teaspoons daily initially
      • 1-2 teaspoons daily in long-term therapy
    • Infants and Toddlers:
      • 1-2 teaspoons daily initially
      • 1 teaspoon daily in long-term therapy
  • Intestinal Flora Disturbances:
    • Adults: 1-2 teaspoons daily
    • Children: 1 teaspoon daily
  • Reduction of Blood Ammonia Level:
    • Hyper-Ammoniemia in Hepatopathy: Maximum of 18-30 teaspoons daily
    • In Portal Systemic Encephalopathy: Hourly doses of 6-9 teaspoons

Interaction

No significant drug interactions with Mylac. The glycosidic effect of cardiac glycosides can be intensified by potassium deficiency in abuse.

Contraindications

  • Hypersensitivity to galactose and/or lactose
  • Galactose-free diet
  • Gastro-cardial symptom complex
  • Suspected intestinal obstruction

SideEffects

  • Occasional flatulence
  • Cramp and abdominal discomfort
  • Diarrhea in overdose
  • Loss of electrolytes, primarily potassium in abuse

PregnancyAndLactation

US FDA Pregnancy Category: B. Lactulose has no adverse effects. Decision on use during pregnancy and lactation should be made by a registered physician.

PrecautionsAndWarnings

Mylac should be administered with care to patients who are intolerant to Mylac. The dose used in the treatment of (pre)coma hepaticum is usually much higher and may need to be taken into consideration for diabetics.

OverdoseEffects

No reports of accidental overdosage. Acute overdosage may result in diarrhea and abdominal cramps.
