Indications
- Treatment of acute and chronic psychoses
- Treatment of schizophrenia
- Bipolar disorder treatment
- Maintenance treatment of bipolar I disorder
- Prevention of recurrence of manic, depressive or mixed episodes
Pharmacology
- Belongs to chemical class of dibenzothiazepine derivatives
- Antagonist at serotonin, dopamine, histamine, and adrenergic receptors
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
- Mechanism of action is unknown
- Efficacy in schizophrenia mediated through dopamine D2 and serotonin 5HT2 antagonism
Dosage & Administration
- Administered twice daily, with or without food
- Total daily dose for first four days of therapy is titrated
- Dosage adjustments based on clinical response and tolerability
- Usual effective dose range 300-450 mg/day for schizophrenia
- Usual effective dose range 400-800 mg/day for manic episodes
- Usual dose for depressive episodes 300 mg/day
Interaction
- Caution when used with medicines causing electrolyte imbalance
- Increases in clearance of Qutap observed with thioridazine, carbamazepine, and phenytoin
Contraindications
- Contra-indicated in patients hypersensitive to Quetiapine
Side Effects
- Common adverse reactions include somnolence, dizziness, dry mouth, weight gain, and extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy during human pregnancy not established
- Use during pregnancy only if benefits justify potential risks
- Avoid breastfeeding while taking Quetiapine
Precautions & Warnings
- Caution for increased risk of suicidal thoughts and self-harm
- Use with caution in patients with cardiovascular disease or predisposition to hypotension
- Risk of seizures and tardive dyskinesia
- Caution in patients with history of seizures
- Discontinue if signs of neuroleptic malignant syndrome appear
- Exercise caution in patients with cardiovascular disease or family history of QT prolongation
Use in Special Populations
- Use with caution in the elderly
- Dosing may need to be slower and lower in the elderly
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses up to 30 grams
- No specific antidote
- Consider possibility of multiple drug involvement in severe intoxication
- Intensive care procedures recommended
- Close medical supervision and monitoring
