Indications
- Active duodenal ulcer
- Active benign gastric ulcer
- Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease
- Zollinger-Ellison Syndrome
- In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell
Dosage
- 20 mg for active duodenal ulcer and active benign gastric ulcer, once daily in the morning
- 20 mg for erosive or ulcerative gastro-esophageal reflux disease, once daily for four to eight weeks
- 20 mg or 10 mg for long-term Gastro-esophageal Reflux Disease maintenance
- 10 mg for symptomatic treatment of moderate to very severe Gastro-esophageal Reflux Disease
- 5 mg or 10 mg for pediatric patients (1 to 11 years of age)
- 60 to 120 mg once daily for Zollinger-Ellison Syndrome
- Combination therapy for Helicobacter pylori eradication
- Dosage adjustment may be necessary based on patient response and condition
Administration
- Should be taken in the morning, before eating
- Tablets should be swallowed whole and not chewed or crushed
Interaction
- Potential interaction with compounds whose absorption is pH dependent
- Co-administration with ketoconazole or itraconazole may result in decreased antifungal plasma levels
- No interaction with liquid antacids observed
- Not recommended to be co-administered with atazanavir
Contraindications
- Hypersensitivity to the active substance or excipients
- Contra-indicated in pregnancy and during breastfeeding
Side effects
- Well-tolerated in short-term and long-term studies
- May cause headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, altered appetite, muscle pain, drowsiness, and dizziness
Pregnancy lactation
- US FDA pregnancy category 'C'
- No adequate and well-controlled studies in pregnant women
- Likely excreted in human milk
- Decision on whether to discontinue nursing should consider the importance of the drug to the mother
Precautions warnings
- Symptomatic response does not preclude malignancy, so malignancy should be excluded prior to treatment
- Regular surveillance for long-term treatment patients
- Possible increased risk of fracture with long-term, high-dose use, especially in the elderly
- Risk of cross-hypersensitivity reactions
- Risk of blood dyscrasias, hepatic enzyme abnormalities, and hypomagnesaemia
- Not recommended for use in children
- Considerations for hypomagnesaemia, vitamin B12 absorption, SCLE, interference with laboratory tests
