Indications

  • Active duodenal ulcer
  • Active benign gastric ulcer
  • Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
  • Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
  • Zollinger-Ellison Syndrome
  • In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease

Pharmacology

  • Suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell

Dosage

  • 20 mg to be taken once daily in the morning for active duodenal ulcer and active benign gastric ulcer
  • 20 mg to be taken once daily for four to eight weeks for erosive or ulcerative GERD
  • Maintenance dose of 20 mg or 10 mg once daily for GERD maintenance
  • 10 mg once daily for symptomatic treatment of moderate to very severe GERD
  • 5 mg once daily for 12 weeks for pediatric patients 1 to 11 years of age (Less than 15 kg)
  • 10 mg once daily for 12 weeks for pediatric patients 1 to 11 years of age (15 kg or more)
  • 60 mg once a day for Zollinger-Ellison Syndrome
  • Combination of rabeprazole sodium 20 mg twice daily, clarithromycin 500 mg twice daily, and amoxicillin 1g twice daily for eradication of H. pylori

Administration

  • Taken in the morning, before eating, and swallowed whole

Interaction

  • May interact with compounds whose absorption is pH dependent, like ketoconazole or itraconazole

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients
  • Contra-indicated in pregnancy and during breastfeeding

Precautions & Warnings

  • Symptomatic response does not preclude the presence of malignancy
  • Regular surveillance for long-term treatment
  • Possible increase in the risk of hip, wrist, and spine fracture
  • Risk of cross-hypersensitivity reactions
  • Potential blood dyscrasias and hepatic enzyme abnormalities
  • Caution in hepatic dysfunction
  • Co-administration of atazanavir not recommended
  • Possibly increased risk of gastrointestinal infections
  • Severe hypomagnesaemia reported in patients treated for at least three months

Use in Special Populations

  • No dosage adjustment necessary for patients with renal or hepatic impairment
  • Not recommended for use in children

Overdose Effects

  • Maximum established exposure has not exceeded 60 mg twice daily, or 160 mg once daily
  • Generally minimal and reversible without further medical intervention
  • No specific antidote known
