Indications
- Active duodenal ulcer
- Active benign gastric ulcer
- Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
- Zollinger-Ellison Syndrome
- In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Suppression of gastric acid secretion
- Inhibition of the gastric H+/K+-ATPase enzyme
- Characterized as a gastric proton-pump inhibitor
Dosage
- Recommended oral dose of 20mg taken once daily in the morning for active duodenal ulcer and active benign gastric ulcer
- Recommended oral dose of 20mg taken once daily for four to eight weeks for erosive or ulcerative GERD
- Maintenance dose of 20mg or 10mg once daily for long-term GERD management
- 10mg once daily for symptomatic treatment of moderate to very severe GERD
- Treatment regimen for pediatric patients 1 to 11 years old based on weight and response
- Adult starting dose of 60mg once daily for Zollinger-Ellison Syndrome with potential titration to 120mg/day
Administration
- To be taken in the morning before eating
- Tablets should not be chewed or crushed, but swallowed whole
Interaction
- Potential interaction with pH-dependent absorption compounds
- Possible decrease in antifungal plasma levels when co-administered with specific medications
- Absorption of atazanavir is pH-dependent and should not be co-administered with rabeprazole
Contraindications
- Hypersensitivity to active substance or excipients
- Contraindicated in pregnancy and breastfeeding
Side Effects
- Well-tolerated overall with common side effects like headache, diarrhea, abdominal pain, vomiting, dry mouth, dizziness
- Rare cases of blood dyscrasias and hepatic enzyme abnormalities reported
Pregnancy & Lactation
- FDA pregnancy category 'C'
- Excretion in human milk, decision should be made regarding continuation based on importance to mother
Precautions & Warnings
- Symptomatic response does not exclude malignancy
- Surveillance for long-term treatment patients
- Modest increase in fracture risk with prolonged use, especially in elderly
- Risk of cross-hypersensitivity reactions
- Caution in severe hepatic dysfunction patients
- Possible hypomagnesaemia and vitamin B12 absorption reduction
- Association with subacute cutaneous lupus erythematosus and interference with certain laboratory tests
Use in Special Populations
- No dosage adjustment for renal or hepatic impairment
- Not recommended for pediatric use

- Effects generally minimal and reversible without intervention
- No specific antidote known, treatment should be symptomatic and supportive
