Indications
- Active duodenal ulcer
- Active benign gastric ulcer
- Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
- Zollinger-Ellison Syndrome
- Eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase
- Characterized as a gastric proton-pump inhibitor
Dosage
- Recommended oral dose for active duodenal ulcer and active benign gastric ulcer is 20 mg once daily in the morning
- Recommended oral dose for erosive or ulcerative GERD is 20 mg once daily for four to eight weeks
- Maintenance dose of rabeprazole sodium can be 20 mg or 10 mg once daily depending upon patient response
- Dosage for pediatric patients: 5 mg or 10 mg once daily for 12 weeks based on weight
- Recommended starting dose for Zollinger-Ellison Syndrome is 60 mg once a day
- Combination therapy for H. pylori eradication: 20 mg rabeprazole sodium twice daily
Administration
- Tablets should be taken in the morning before eating
- Tablets should not be chewed or crushed, but should be swallowed whole
Interaction
- Potential interaction with ketoconazole, itraconazole, and atazanavir
- No interaction with liquid antacids observed
Contraindications
- Hypersensitivity to the active substance or excipients
- Contraindicated in pregnancy and during breastfeeding
Side Effects
- May cause headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, altered appetite, muscle pain, drowsiness, dizziness
Pregnancy & Lactation
- US FDA pregnancy category 'C'
- No adequate and well-controlled studies in pregnant women
- Likely excreted in human milk
Precautions & Warnings
- Malignancy should be excluded prior to commencing treatment
- Close surveillance for long-term treatment patients
- Possible increased risk of hip, wrist, and spine fracture
- Risk of cross-hypersensitivity reactions with other proton pump inhibitors
- Care needed for patients at risk of osteoporosis
- Potential for hypomagnesaemia, hypovitaminosis B12, and cutaneous lupus erythematosus
- Potential interference with laboratory tests
Use in Special Populations
- No dosage adjustment for renal or hepatic impairment
- Not recommended for use in children
Overdose Effects
- Maximum established exposure not exceeding 60 mg twice daily or 160 mg once daily
- Effects generally minimal, reversible without further medical intervention
- No specific antidote known, treatment should be symptomatic and supportive
