Indications
- Active Duodenal Ulcer
- Active Benign Gastric Ulcer
- Symptomatic Erosive or Ulcerative Gastro-esophageal Reflux Disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe Gastro-esophageal Reflux Disease (symptomatic GERD)
- Zollinger-Ellison Syndrome
- In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease
Mode of Action
- Suppresses gastric acid secretion
- Inhibits the gastric H+/K+-ATPase
Dosage
- 20 mg recommended oral dose for active duodenal ulcer and active benign gastric ulcer
- 20 mg recommended oral dose for erosive or ulcerative gastro-esophageal reflux disease (GERD)
- 20 mg or 10 mg maintenance dose for GERD long-term management
- 10 mg recommended oral dose for symptomatic treatment of moderate to very severe GERD
- 5 mg once daily for pediatric patients 1 to 11 years of age (Less than 15 kg)
- 10 mg once daily for pediatric patients 1 to 11 years of age (15 kg or more)
- 60 mg once daily recommended adult starting dose for Zollinger-Ellison Syndrome
- Rabeprazole sodium 20 mg twice daily, clarithromycin 500 mg twice daily, and amoxicillin 1g twice daily for H. pylori eradication
Administration
- To be taken in the morning before eating
Interaction
- May interact with compounds whose absorption is pH dependent
- Co-administration with ketoconazole or itraconazole may decrease antifungal plasma levels
- No interaction with liquid antacids observed
- Absorption of atazanavir is pH-dependent, should not be co-administered with PPIs
Contraindications
- Hypersensitivity to the active substance or excipients
- Contra-indicated in pregnancy and during breastfeeding

Precautions & Warnings
- Symptomatic response does not preclude the presence of gastric or esophageal malignancy
- Regular surveillance for long-term treatment
- May modestly increase the risk of hip, wrist, and spine fracture
- Risk of cross-hypersensitivity reactions with other PPIs
- Hypomagnesaemia risk
- Influence on vitamin B12 absorption
- Association with subacute cutaneous lupus erythematosus (SCLE)
- Interference with laboratory tests
Use in Special Populations
- No dosage adjustment is necessary for patients with renal or hepatic impairment
- Not recommended for use in children due to a lack of data on safety and efficacy
Overdose Effects
- Maximum established exposure not exceeded 60 mg twice daily or 160 mg once daily
- Generally minimal effects, reversible without further medical intervention
- No specific antidote known
- Not dialysable
