Dosage

  • 20 mg once daily in the morning for active duodenal ulcer and active benign gastric ulcer
  • 20 mg once daily for four to eight weeks for erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • 20 mg or 10 mg once daily for long-term management of GERD
  • 10 mg once daily for symptomatic treatment of moderate to very severe gastro-esophageal reflux disease
  • 5 mg once daily for pediatric patients 1 to 11 years of age (Less than 15 kg)
  • 10 mg once daily for pediatric patients 1 to 11 years of age (15 kg or more)
  • 60 mg once daily for Zollinger-Ellison Syndrome, may be titrated upwards to 120 mg/day based on patient needs
  • 20 mg twice daily in combination with antibiotics for Helicobacter pylori eradication

Indications

  • Active duodenal ulcer
  • Active benign gastric ulcer
  • Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
  • Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
  • Zollinger-Ellison Syndrome
  • Helicobacter pylori eradication in patients with peptic ulcer disease

Administration

  • To be taken in the morning, before eating
  • Tablets should not be chewed or crushed, but swallowed whole

Interaction

  • Potential interaction with pH-dependent compounds
  • Co-administration with ketoconazole or itraconazole may decrease antifungal plasma levels
  • No interaction with liquid antacids observed
  • Co-administration with atazanavir not recommended

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Contra-indicated in pregnancy and during breastfeeding

  • Well-tolerated
  • Common: headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, changes in appetite, muscle pain, drowsiness, dizziness

  • No adequate and well-controlled studies in pregnant women
  • Drug likely to be excreted in human milk
  • Risk and benefit analysis advised for breastfeeding mothers

Precautions & Warnings

  • Symptomatic response to therapy does not exclude the presence of gastric or esophageal malignancy
  • Regular surveillance for patients on long-term treatment
  • Possible increase in risk of hip, wrist, and spine fracture with long-term use
  • Risk of cross-hypersensitivity reactions
  • Caution in patients at risk of osteoporosis
  • Reports of blood dyscrasias and hepatic enzyme abnormalities
  • No clinical data on use in patients with severe hepatic dysfunction
  • Possible increase in risk of gastrointestinal infections
  • Hypomagnesaemia may occur after prolonged use
  • Reduction in vitamin B12 absorption due to hypo- or a-chlorhydria
  • Rare association with subacute cutaneous lupus erythematosus (SCLE)
  • Interference with investigations for neuroendocrine tumors
  • Caution in patients with reduced body stores of vitamin B12 or risk factors for reduced absorption
  • Avoid long-term use in children

Use in Special Populations

  • No dosage adjustment necessary for patients with renal or hepatic impairment
  • Not recommended for use in children

Overdose Effects

  • Maximum established exposure has not exceeded specified doses
  • Effects generally minimal and reversible
  • No specific antidote known
