Indications
- Treatment of active duodenal ulcer
- Treatment of active benign gastric ulcer
- Treatment of symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Long-term management of gastro-esophageal reflux disease (GERD maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
- Treatment of Zollinger-Ellison Syndrome
- Eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Suppresses gastric acid secretion by inhibiting the gastric H⁺/K⁺-ATPase at the secretory surface of the gastric parietal cell
Dosage
- Recommended oral doses for different conditions like active duodenal ulcer, active benign gastric ulcer, erosive or ulcerative gastro-esophageal reflux disease (GERD), GERD maintenance, symptomatic GERD, Zollinger-Ellison Syndrome, and Helicobacter pylori eradication
Administration
- Should be taken in the morning before eating, not to be chewed or crushed, to be swallowed whole
Interaction
- May interact with compounds whose absorption is pH dependent, interaction with ketoconazole or itraconazole resulting in decreased antifungal plasma levels, no interaction with liquid antacids observed, not recommended for co-administration with atazanavir
Contraindications
- Hypersensitivity to the active substance or excipients, contraindicated in pregnancy and during breastfeeding
Side Effects
- Possible side effects include headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, increased or decreased appetite, muscle pain, drowsiness, dizziness
Pregnancy & Lactation
- US FDA pregnancy category 'C', no adequate and well-controlled studies in pregnant women, likely to be excreted in human milk
Precautions & Warnings
- Symptomatic response to therapy does not exclude the presence of gastric or esophageal malignancy, patients on long-term treatment should be kept under regular surveillance, may modestly increase the risk of hip, wrist, and spine fracture in high doses and over long durations, risk of cross-hypersensitivity reactions with other proton pump inhibitor or substituted benzimidazoles cannot be excluded
Use in Special Populations
- No dosage adjustment necessary for patients with renal or hepatic impairment, not recommended for use in children due to lack of data on safety and efficacy
Overdose Effects
- Maximum established exposure, effects generally minimal and reversible without further medical intervention, no specific antidote known, not dialysable, treatment should be symptomatic and general supportive measures should be utilized
