Indications

  • Active duodenal ulcer
  • Active benign gastric ulcer
  • Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
  • Zollinger-Ellison Syndrome
  • Eradication of Helicobacter pylori in patients with peptic ulcer disease

Pharmacology

  • Suppresses gastric acid secretion
  • Inhibits the gastric H+/K+-ATPase
  • Characterized as a gastric proton-pump inhibitor

Dosage

  • 20 mg once daily for active duodenal ulcer and active benign gastric ulcer
  • 20 mg once daily for four to eight weeks for erosive or ulcerative GERD
  • 20 mg or 10 mg once daily for GERD maintenance
  • 10 mg once daily for symptomatic GERD
  • 60 mg once a day for Zollinger-Ellison Syndrome

Administration

  • Should be taken in the morning before eating
  • Tablets should not be chewed or crushed, but swallowed whole

Interaction

  • May interact with compounds whose absorption is pH dependent
  • Co-administration with ketoconazole or itraconazole may decrease antifungal plasma levels
  • No interaction with liquid antacids

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Contra-indicated in pregnancy and during breastfeeding

Precautions & Warnings

  • Exclude the possibility of gastric or esophageal malignancy prior to treatment
  • Regular surveillance for long-term treatment
  • Possible increase in risk of hip, wrist, and spine fracture
  • Risk of cross-hypersensitivity reactions
  • Hypomagnesaemia reported after prolonged treatment
  • Reduction in absorption of vitamin B12
  • Possible association with subacute cutaneous lupus erythematosus
  • Interference with laboratory tests such as Chromogranin A levels

Use in Special Populations

  • No dosage adjustment necessary for renal or hepatic impairment
  • Not recommended for use in children due to lack of data on safety and efficacy

Overdose Effects

  • Maximum established exposure not exceeding 60 mg twice daily or 160 mg once daily
  • Effects are generally minimal, reversible, and representative of known adverse event profile
