Indications
- Hypertension
- Congestive heart failure
- Treatment after myocardial infarction

Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Decreased plasma angiotensin II
- Antihypertensive activity
- Management of heart failure
- Risk reduction of stroke and myocardial infarction
Dosage
- Adjusted according to patient tolerance and response
- Initial dose of 1.25-2.5 mg once daily for hypertension
- Maintenance dosage of 2.5-20 mg daily
- Initial dose of 2.5 mg twice daily for heart failure after myocardial infarction
- Prevention of major cardiovascular events: first week at 2.5 mg once daily, following 3 weeks at 5 mg once daily
- Titration for renal impairment
Administration
- Tablets to be swallowed with sufficient liquid
- Absorption unaffected by food
- Can be taken before, during, or after a meal
Interaction
- Concomitant administration with diuretics may lead to serious hypotension
- Dangerous hyperkalemia with potassium sparing diuretics
- Increased serum lithium concentration with lithium therapy
- Reduction in BP affecting ability to drive
- NSAIDs reducing antihypertensive effect and causing deterioration of renal function
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines or AIIRAs in specific patients
- During pregnancy
- Concomitant use with certain extracorporeal treatments

Pregnancy & Lactation
- Must not be taken during pregnancy
- Pregnancy to be avoided where treatment with ACE inhibitors is indispensable
- Discontinuation and replacement in case of pregnancy during treatment
- Not recommended during breastfeeding
Precautions & Warnings
- Caution in patients with impaired renal function
- Caution in patients with hyperkalaemia
- Caution in patients with hypotension
- Caution in patients with impaired hepatic function
Use in Special Populations
- Elderly: reduced initial dose
- Hepatic impairment: initiation under close medical supervision
- Renal impairment: dosage adjustment
- Patients pretreated with a diuretic: consideration for discontinuation or reduction
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension, shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
- Management: gastric lavage, administration of adsorbents, electrolyte management, volume and salt substitution
