Indications
- Lowering blood pressure
- Managing congestive heart failure
- Treating post-myocardial infarction patients
- Treating glomerular or incipient nephropathy
- Reducing the risk of cardiovascular events
Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity
- Decreases aldosterone secretion
- Long-acting and well-tolerated
Dosage
- Adjustable based on patient tolerance and response
- Initial dose for hypertension in adults is 1.25-2.5 mg once daily
- Initial dose for congestive heart failure after myocardial infarction is 2.5 mg twice daily
- Initial dose for prevention of major cardiovascular events is 2.5 mg once daily
- Dosage may be increased as tolerated based on renal impairment
Administration
- Tablets must be swallowed whole
- Absorption not significantly affected by food
- Can be taken before, during, or after a meal
Interaction
- May lead to serious hypotension when taken with diuretics
- May increase serum lithium concentration when taken with lithium
- Reduction in blood pressure may affect ability to drive or operate machinery
- NSAIDs may reduce antihypertensive effect and cause deterioration of renal function
Contraindications
- Hypersensitivity to ramipril or other ACE inhibitors
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines in diabetics or those with moderate to severe renal impairment
- Concomitant use with angiotensin II receptor antagonists (AIIRAs) in patients with diabetic nephropathy
- During pregnancy

- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalaemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Reduced initial dose recommended for the elderly
- Initiation in hepatic impairment under close medical supervision
- Adjusted doses in renal impairment
- Consideration for patients pretreated with a diuretic
Overdose Effects
- May cause excessive peripheral vasodilatation
- Marked hypotension
- Bradycardia
- Electrolyte disturbances
- Renal failure
- Primary detoxification methods
- Administration of adrenergic agonists or angiotensin II in case of hypotension
