Indications
- Treatment of chronic hepatitis C virus infection
- Combination therapy with other antiviral drugs
- Patients with compensated liver disease
- Adult CHC patients coinfected with HIV
Pharmacology
- Inhibitory action against respiratory syncytial virus
- Inhibitory action against influenza virus
- Inhibitory action against herpes simplex virus
- Mechanism of action not entirely clear
- Interference with viral nucleic acid synthesis
Dosage & Administration
- Recommended duration of treatment for previously untreated patients: 24-48 weeks
- Assessment of virologic response after 24 weeks
- Treatment discontinuation if HCV RNA not below the limit of detection by 24 weeks
- No safety and efficacy data for treatment longer than 48 weeks for previously untreated patients
- Recommended duration of treatment for patients that relapse following interferon therapy: 24 weeks

Interaction
- Potential interactions with nucleoside reverse transcriptase inhibitors

Contraindications
- Contraindicated in pregnant women
- Potential for fetal harm
- Contraindicated in patients with hemoglobinopathies
- Contraindicated in combination with didanosine
- Reports of fatal hepatic failure in clinical trials
Side Effects
- Common psychiatric and central nervous system effects in adults
- Severe ocular and dental disorders
- Growth inhibition in children and adolescents
- Irreversible growth inhibition in some patients
- Similar adverse reactions in pediatric subjects as in adults
Pregnancy & Lactation
- Category X in pregnancy
- Significant embryocidal and teratogenic effects in animal studies
- Reduction in survival of fetuses and offspring
- Unknown excretion of Ribavirin in human milk
- Potential for serious adverse reactions in nursing infants
Precautions & Warnings
- Avoid use in pregnancy and for 6 months after treatment
- Requirement for negative pregnancy test prior to therapy
- Use of at least 2 forms of contraception and monthly pregnancy tests
- Importance of avoiding pregnancy during treatment and post-treatment period
- Warning for male patients to avoid impregnating female partner during and after treatment
Use in Special Populations
- Safety and effectiveness not established in pediatric patients below the age of 3 years
- Greater risk of toxic reactions in patients with impaired renal function
- Dose adjustments based on creatinine clearance
