Indications
- Treatment of chronic hepatitis C (CHC) virus infection in combination with other antiviral drugs
- For patients with compensated liver disease not previously treated with interferon alpha
- For adult CHC patients coinfected with HIV
Pharmacology
- Inhibitory action against respiratory syncytial virus, influenza virus and herpes simplex virus
- Mechanism of action is not clear
- Interference with viral nucleic acid synthesis
Dosage & Administration
- Recommended duration of treatment for previously untreated patients is 24 to 48 weeks
- Assessment of virologic response after 24 weeks of treatment
- No safety and efficacy data for treatment exceeding 48 weeks in previously untreated patients
- Recommended duration of treatment for patients who relapse following interferon therapy is 24 weeks
- No safety and efficacy data for treatment exceeding 24 weeks in relapse patient populations
- Different dosage for different genotypes and patient weights
Interaction
- Potential interaction with nucleoside reverse transcriptase inhibitors
- Possible increase in risk of azathioprine-related myelotoxicity with concomitant use of Viracin and azathioprine
Contraindications
- Contraindicated in pregnant women
- Avoid use in women who may become pregnant during treatment
- Contraindicated in patients with hemoglobinopathies
- Contraindicated in combination with didanosine
Side Effects
- Common adverse reactions in adults receiving combination therapy include psychiatric and central nervous system effects, ocular disorder, dental and periodontal disorders, and growth inhibition in children and adolescents
- Similar adverse reactions in pediatric subjects as seen in adults
Pregnancy & Lactation
- Pregnancy Category X due to significant embryocidal and/or teratogenic effects in animal studies
- Not known whether Ribavirin is excreted in human milk
- Decision should be made to discontinue nursing or therapy with Ribavirin based on the importance of the therapy to the mother
Precautions & Warnings
- Do not use in pregnancy and for 6 months after treatment
- Female patients must have a negative pregnancy test prior to therapy, use at least 2 forms of contraception, and undergo monthly pregnancy tests
- For male patients, it is important for their female partners to avoid becoming pregnant during treatment and during the 7 months after treatment, and to avoid intercourse with pregnant women
Use in Special Populations
- Safety and effectiveness in combination with Peginterferon not established in pediatric patients below the age of 3 years
- Greater risk of toxic reactions in geriatric patients with impaired renal function
- Dosage adjustment based on creatinine clearance
